The history of the town dates back to the 10th century. The first documented mention of the town dates back to 995, when Emperor Otto III stayed at the royal hunting lodge "Elysinaburg" and issued a charter for the already existing Drübeck monastery. Ilsenburg Monastery was then officially founded in 1018.

Over the centuries, the bailiff in charge changed several times, including to the Counts of Wernigerode. The peasant uprisings in 1525 brought the monastery's time to an end. The history of Ilsenburg Castle begins with the dissolution of the monastery convent. The resulting village developed into a hamlet and eventually into a prosperous community.

Ilsenburg was first certified as a climatic health resort in 1947. The town charter was granted in 1959. The town's more recent history includes the incorporation of the municipalities of Darlingerode and Drübeck with the district of Öhrenfeld in 2009.
